Medical Concerns Details

Thank you for registering your child / children for Religious Education classes. In-person classes requires follow-up on medical concerns and allergies noted on registration forms.

Diocesan required medical forms and medications must be received at least one week prior to the start of class for all medications required during RE classes or activities.
